Why improvement programs stall at implementation
The analysis is usually right. What is missing is an owner, a delivery structure, and a measure that survives the handoff.

Ask a leadership team what their top operational problems are and most can name them accurately. Ask what has measurably changed in the last two quarters and the room gets quieter. The gap is rarely diagnostic. It is executional.
Three common failure points
- Improvement is owned by a team with no authority to implement
- Delivery is treated as an administrative wrapper instead of the work itself
- The measure changes definition somewhere between the business case and the review
The fix is structural, not motivational. Define the baseline before the work begins, give the initiative a delivery owner with decision rights, and agree in advance what evidence will count as success six months after go-live.
Sustainment is designed at the start
Control plans, named owners, and operating cadence are not closing activities. When they are designed in the improve phase, adoption is a consequence of the design rather than a campaign at the end of it.
